Step 2: Completing the form.
Please click on each field (e.g. Candidate's First Name) and type the appropriate
text. To mark a check box, simply click on the appropriate selection, and a
checkmark should appear in the box.
Step 3: Printing the form.
When you have finished filling out the form, you will need to print it. Please click
